INFORMATION CLAUSE
In accordance with Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC, hereinafter referred to as: Regulation 2016/679, we inform you that:
1) The controller of your personal data is VIA-POL Mateusz Wiatrzyk based in Tychy (43-100), ul. Piłsudzkiego 12/420 (hereinafter referred to as a Controller);
2) Your personal data will be processed on the basis of Article 6(1)(a) of Regulation 2016/679, i.e. on the basis of your consent, in order to send commercial information regarding the Controller’s services and products;
3) We entrust personal data for processing, among others, to: suppliers providing IT support, hosting service providers;
4) Your personal data will not be transferred to a third country;
5) Your personal data will be processed for a period of up to […] days from the date of sending the commercial information;
6) You have the right of access to the content of your data and the right to rectification, erasure, restriction of processing, right to data portability, right to object, right to withdraw consent at any time without affecting the compliance of the processing carried out on the basis of consent prior to its withdrawal;
7) You have the right to file a complaint with the supervisory authority – the President of the Office for Personal Data Protection – if you think that the processing of your personal data violates the provisions of Regulation 2016/679;
8) The provision of personal data is voluntary, but necessary to send commercial information on the Controller’s services and products; 9) Your data will not be processed automatically and will not be subject to profiling.
